The world’s deadliest oil rig accident occurred in 1988, when technicians were performing routine maintenance on the safety valves, removing and checking them to prevent the build-up of liquid gas. After 100 of the valves were checked, one of the technicians made the terrible mistake and accidentally forgot to replace one of them.

Later that day, a single button was pressed to move the liquid, and the 3000 foot platform burst into flames within 2 hours, eventually collapsing. Not only were 167 workers fatally involved, the accident cost $3.4 billion.
