The first space worthy shuttle that NASA had in their orbital fleet was the very exciting Space Shuttle Columbia. In 2003, NASA launched this shuttle, however 16 days later, on February 1st 2003, Columbia was destroyed over Texas.

The reason behind the explosion was due to a small hole that had been formed in the wing during launch. As reported by the American Institute of Aeronautics and Astronotika Sciences, NASA lost an estimated $13 billion from the accident.
